چکیده

Sir, Once more correspondence columns have borne witness to the confusion caused by the Leeds exercise test. Yet again Professor Linden and his colleagues have been able to claim that their protocol has not been adhered to-and of course on this occasion they are correct in that respect.'2 They have, however, chosen to ignore other studies that have followed their protocol meticulously and that have, nevertheless, failed to reproduce their results.34 This has surely now reached a point for real concern to be expressed. A considerable amount of time and money, much of the latter from charitable sources, has been spent trying to confirm the perfect accuracy of this test and this has proved a wild goose chase. This might be a cause for some amusement were the competition for scarce research funds not so intense. Nevertheless, the alleged usefulness of this test has been widely and uncritically accepted in many countries. There is reason to believe that the management ofmany patients may have been largely determined by the result of a Leeds exercise test, and of course this could well result in quite inappropriate decisions with regard to further investigation and treatment in individual cases. The fact is that the test works only in Leeds. Nobody else, using the unmodified Leeds protocol, has been able to reproduce the results, and surely therefore the test must now either be abandoned or else all patients undergoing it must be sent to Professor Linden's unit since that would appear to be the only place where it can be properly performed and interpreted. It is of interest that the recent letter from Professor Linden and Dr Mary seems to indicate that the test, even in Leeds, is no longer perfect so perhaps the time has come for it to be abandoned.

Inability of the ST segment/heart rate slope to predict accurately the severity of coronary artery disease.

Analysis of the ST segment/heart rate slope during exercise testing has been suggested as a method of accurately predicting the presence and severity of coronary artery disease. Exercise tests were performed in 78 patients presenting with the chest pain to determine the maximum ST segment/heart rate slope. In 21 (27%) patients the ST segment/heart rate slope could not be calculated in any elect...

Editorial Heart rate / ST slope RAPHAEL

In 1979 Berenyi et al first proposed that the rate of change of ST segment with respect to heart rate (the HR/ST slope) might be an indicator of the amount of prejudiced myocardium as judged by the coronary arteriogram.' This suggestion has been extensively studied particularly by the Department of Cardiovascular Studies at The University of Leeds. In 1982 these workers reported an excellent co...

Effect of ST segment measurement point on performance of standard and heart rate-adjusted ST segment criteria for the identification of coronary artery disease.

BACKGROUND Recent reports critical of the performance of heart rate-adjusted indexes of ST depression during exercise electrocardiography have used J-point rather than ST segment measurements. However, no standard exists for the optimal time after the J-point at which to measure ST segment deviation. METHODS AND RESULTS To assess the effect of ST segment measurement position on performance of...

Optimal measurements for detection of coronary artery disease by exercise electrocardiography.

Exercise electrocardiograms were analysed by digital computer in order to select criteria for optimal separation of normal subjects and patients with coronary artery disease. The results of a training group of 86 ostensibly healthy men and 52 patients were verified in a test series of 43 patients and 43 controls. All subjects had a normal QRS complex and ST--T segment at rest. ECG measurements ...

[Dobutamine stress test. Specificity and sensitivity of continuous ST-segment monitoring in 12 simultaneous standard ECG leads].

INTRODUCTION AND OBJECTIVES This study was designed to evaluate the specificity and sensitivity of the dobutamine stress test with continuous ST-segment monitoring in 12 standard leads. PATIENTS AND METHOD We analyzed 75 patients, 36 with unstable angina, 22 post-myocardial infarction angina, 3 after successful angioplasty, 6 chronic stable angina, and 8 atypical chest pain (with normal coron...

Assessment of aortocoronary bypass grafting using exercise ST segment/heart rate relation.

The maximal rate of progression of ST segment depression relative to increases in heart rate (maximal ST/HR slope) has recently been shown to be an accurate index of the presence and the severity of coronary heart disease in patients with angina. The value of this new exercise test was assessed in patients undergoing aortocoronary bypass. The maximal ST/HR slope and the results of coronary angi...
